A medical record is maintained for every individual evaluated and treated at Flushing Hospital Medical Center.

For your convenience, we have provided several options to request a copy of your medical records. A copy can be requested online, at our Medical Records Department or by mail. The confidentiality of medical and personal information is a right of every patient and information shall not be released without the proper written authorization of the patient, legal guardian, executor of the estate, or court order.

Please include a copy of a valid photo ID with your completed HIPAA authorization to verify your identity and protect your privacy.

Acceptable ID includes a driver’s license, state-issued ID, passport, or other government issued photo identifications.

There is a fee for copies of medical records, unless the information is going directly to a physician. All completed forms should be brought or mailed to:

You can also visit the Medical Records Department, which is located on the first floor adjacent to the main lobby. The department is open Monday to Friday, from 9 AM to 5 PM, and can be reached by calling 718-670-5424.

You can access information about birth certificate records or obtain copies by contacting the NYC Bureau of Vital Records.

How to Obtain Your Child’s School Health Summary

Use Medisys Connect to download your child’s School Health Summary. After signing in, select your child’s chart (proxy access required). From the Sharing Hub, scroll to the bottom of the page and click Download School Health Summary to request a copy of the report. This feature is available for patient age 11 and under.
